Merhaba,
Bu hafta blog yazımda keyifli bir konu ile karşınızdayım. Son dönemlerde egzersiz farkındalığı giderek artmaktadır. Artan bu egzersiz farkındalığı ile beraber birçok kişide (cinsiyetten bağımsız bir şekilde) kaslanma güdüsü baskın hale gelmiştir. Kaslanma isteği büyük oranda estetik kaygılar ve iyi görünme gibi niyetler için istense de genel sağlığı korumak ve sürdürmek için de yaygın bir şekilde pratiklenmektedir. Bu bağlamda, blog yazımda odaklanmak istediğim konu temelde kas kütlesi ile sağlık hatta yaşam beklentisi arasında ilişki olup olmadığıdır? Peki gerçekten kas kütlesi azalması yaşam beklentisini kısaltabilir mi? Gelin beraber bu soruya cevap arayalım. Öncelikle kas kütlesini nasıl belirleyebileceğimizi de bence netleştirmek yararlı olacaktır. Yağsız vücut ağırlığının belirlenmesinde pahalı ve sofistike yöntemler kullanılabilmesine rağmen (bodpod, dexa, BIA vb.) kolay ve ekonomik yaklaşımlar da bulunmaktadır. Örneğin, kabaca vücut ağırlığından vücut yağ ağırlığının çıkarılması ile yağsız vücut ağırlığı hesaplanabilir.
Yağsız Vücut Ağırlığı= Vücut ağırlığı – Yağ kütlesi
Bu noktadan sonra kas kütlesinin yağ kütlesine oranla gerçekten protektif (koruyucu) bir rolü olup olmadığına bilimsel kanıtlar eşliğinde yanıt oluşturalım. Öncelikle yağ kütlesi veya oranının artışı tüm ölüm risk faktörleri arttırmada etkili olduğu yönünde genel bir konsensüs bulunmaktadır (1,2). Bu bağlamda, yağ kütlesi artışı tüm hastalıklar kaynaklı ölüm riskini arttırma potansiyeli olan iyi dokümante olmuş bir fenomendir. Bu durumun tam aksine kas kütlesi artışı ise hem toplam sağlığı korumak ve sürdürmek hem de hastalıkları önlemek için etkili bir strateji olabilir. Bazı bilimsel araştırmalar, kas kütlesi artışı ile metabolik hastalıkların görülme oranı ve mortalite (ölüm) riski arasında negatif bir ilişki olduğu yönünde kanıtlar içermektedir (3,4,5). Dahası bu araştırmalar büyük oranda bel/kalça oranı ile hem sağlık parametreleri hem de hastalık gelişim riski arasında bağlantı olabileceğini de iddia etmişlerdir. Ayrıca, kalça bölgesinde bulunan kasların kütlesinin fazlalığı yaşlı popülasyonlar için tüm ölüm risk faktörlerini minimize etmede daha etkili olabileceği yönündedir (6).
Bilimsel çalışmalar konu sağlık ve uzun yaşam olduğunda yağ yakma stratejilerinden ziyade kas kütlesi artışının daha etkili olduğunu savunmaktadır. Bu kanının altında yatan rasyonel neden ise kas kütlesinin azalması beraberinde kaslara giden kan akışında azalma, kan şekeri regülasyonunun bozulması, kardiyak çıktının düşmesi ve ateroskleroz damar hastalıkları gelişimi gösterilmiştir (7,8,9). Tüm bu bulgular ışığında kas kütlesi azalmasının yaşlı popülasyonlarda tüm faktörlere bağlı ölüm riskini arttırabileceği vurgulanmıştır.
Sonuç olarak, uzun yıllardır sağlığın korunması ve sürdürülmesi için hatta tüm hastalıklara bağlı ölüm riskini düşürmede yağ kütlesi oranına veya zayıflama stratejilerine odaklandır. Ancak, bu bakış açısından sanılanın aksine çok fazla işe yaramadığı dahası sorunu önlemede yetersiz kaldığı düşünülmektedir. Bu bakış açısı yerine bireylerin kas kütlelerini arttırmaya yönelik oluşturulan stratejilerin hem toplam sağlığı geliştirme ve sürdürme hem de tüm ölüm risk faktörlerini minimize etmede (sağkalım süresi optimizasyonu) daha efektif olacağı yönünde genel bir kabul oluşmuştur. Bu bağlamda, hemen her yaş, cinsiyet ve seviyede kişiler için haftada 2-3 kez planlı yapılandırılmış direnç egzersizi yapmak son derece rasyonel bir bakış açısı olacaktır.
